Rates/Insurance

Dr. Ghafoor is out-of-network with all insurance plans. I will provide you a superbill (receipt) after your appointment(s) that you can submit to your insurance for out-of-network reimbursement. Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychiatric services.

I would recommend asking these questions to your insurance provider to help determine your benefits:

  • Does my health insurance plan include mental health benefits?
  • Do I have a deductible? If so, what is it and have I met it yet?
  • What percentage of out-of-network services does my plan reimburse?
  • Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
  • Do I need written approval from my primary care physician in order for services to be covered?

Payment

All payment is through the patient portal and taken at the completion of your appointment. A credit card is held on file – this is to help limit the time spent on administrative tasks.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 48 hours beforehand. You can send a message through the portal to notify me of the cancellation. Otherwise, you will be charged for the full rate of the session.
